Free registry cleaners only do clean up. They can’t fix registry structure or defragment it.
A full-featured registry maintenance tool will do clean up, structure maintenance and registry defragging, but you’re pretty much going to have to pay for such a tool. The free ones only do a part of all that can be done to optimize your registry.
I personally use Uniblue’s Registry Cleaner http://www.liutilities.com/products/registrybooster/
These are all excellent registry cleaners. I have used them all in conjunction with each other for years without a single mishap. You’d be surprised on how many registry entries they can clean up, even on a fresh install of Windows.

I use Glary Utilities and it is a great little program. The 1-click clean-up feature is really useful and I highly recommend it. The registry cleaner is pretty good (not the best), BUT it is completely SAFE, which is most important! (It even does an automatic, registry backup.) As far as a one-click, set-it and forget it maintenance program, this is going to be a pretty good one. Other people may recommend Advanced Windows Care (free), but I personally like this better. It has more features, like a disk cleaner module, a memory-optimizer, an undelete module, etc. I think this is going to be about the top of the line as far as freebie maintenance programs.
