They look for things that should not be there or things that should be there but not.

They basically go through the registry and look for entries that point to files that do not exist.
Some program may not uninstall completely, some registry are left. so the registry files get bigger and fragmented, this will slowly down the system.
The registry cleaner will do the following :
1) scan all system for registered entries.
2) scan all valid programs, list of valid extensions, list all valid temporary files, and recent documents.
3) cross-check within files and program, for orphan-registry it will deleted.
4) deleting invalid entries, then defragment the registry, make it compact and smaller, so system will access it faster.
