No, they are not the same thing. IMO, anti-virus software is much more important to have running on a permanent basis. A registry cleaner is something you can and should run every now and then to keep the registry cleaner from any errors that may be slowing down your PC. But don’t confuse the two, they are different.
If you dont have anti-virus software protecting your PC, you NEED to get this ASAP!
If you are lookin for the best registry cleaner, I would personally recommend software called, RegCure. It is made by a company called ParetoLogic and has worked great for me over the past few months. You can run a free scan at http://www.registryrepairtips.com/ This will let you know if you actually do have any errors. If you do, you can then choose to resolve them by purchasing the software.

Good luck!
An AV is a must.
A registry clean-up software is not.
registry clean-up software does not replace an AV.
No. Registry cleaners remove uneeded or junk files from the registry. I would recommend ccleaner from http://ccleaner.com/ to do this.
AV is a must unless you are very experienced, but is not the only thing you need. I use this combination of free software:
Avira: http://www.free-av.com/ (better than Norton or AVG)
Spyware Terminator (turn off web security guard): http://www.spywareterminator.com/
Spybot: http://www.safer-networking.org/ (use immunise feature)
Superantispyware: http://www.superantispyware.com/
Comodo: http://comodofirewall.notlong.com/ (turn off defense+ to see fewer alerts)
