They look for obsolete information, things like references to files or locations that are no longer on your computer. Then they will call it an "error" or "invalid this or that" to make it sound like they are fixing something. They do not scan your entire registry, even though they might say they do. They can help in cleaning up stuff that programs leave behind when they are uninstalled. If you install and uninstall a lot there’s going to lots of useless information in the registry. Who knows if cleaning some of it out really does anything.
They clean things like Internet Explorer & Firefox history, temp internet files and cookies which frees up space and can improve performance.

If you don’t have an understanding of how the registry works, best not to use any registry cleaners, as they can do more harm then good if you don’t know what it’s doing. The registry is basicly how the computer functions, if something gets messed up in the registry your computer will get errors, blue screen errors and not work correctly.
Keys which point to file extensions which are not used (empty) for example, keys which point to files and locations which do not exist, et cetera.
registry cleaners simply search for shortcut to files that no longer exist, due to the app or file being deleted, you’d be surprised how many of these there are.
make sure you use a reputable registry cleaner and always make a backup first
