My father’s computer is very slow and it makes a grinding noise when it has to process information. I’m pretty sure it’s not a problem on the software end. I think maybe the RAM is toast or the hard drive damaged. Is there a diagnostic tool in XP (or maybe in BIOS) that can run checks on my hardware to see if they are performing as they should?

Western Digital supplies a free hard drive diagnostic, go to their site. Sounds like a hard drive dying, diagnose that first.
If the pc is making a physical noise when the c drive is processing the information, it may be possible that the drive "Bearings" may be going bad. This also maybe a precursor to clean out the computer for dust.
What you can do is open the case before you power up and find out exactly where this noise may be comming from. It could be also the CPU fan or the power supply fan as well. Just remember to attach a grounding wire to your wrist before removing the case, Electro static discharge is really nasty this time of the year. And since you have the case open, might as well and remove the dust out of there as well.
